If registering more than one person, select the number of “Spaces”, and enter the name and email of each attendee in the appropriate field.    In the “Contact Name” and “Contact email” field, enter the main contact point person (usually you).

If you don’t have a PayPal account, you can STILL pay with a Credit Card.  Select PayPal, and then on the next screen for PayPal, the bottom option is to pay with a credit card (versus signing on to PayPal).

If you want to be invoiced,  in the “Pay With” Field, please select “Pay via Check/Invoice”  and and invoice will be e-mail to you automatically to the email address you supply.  If you do not receive one, please let us know.

COSTS

•  $55 – Early Registration Discount (payment received before Dec 23)
•  $65 – Full Price Registration (payment received between Dec 23 – January 31)
• $75 – Walkup Registration (Walkup Registration is not Guaranteed.  Last we had to close walkups as we were already full.   If you want to come, signup!)
•  $500 – Full Coaching Staff Discount (10 to 15 coaches – must  register/pay before Jan 15.    Select the number of Coaches who are attending below. Fill out the name/email for each coach. Then, at the bottom of the page, select “Pay via Check/Invoice” and we will email you the details for payment submission and processing. If you would like to bring more than 15 coaches, please email capcityfbclinic@gmail.com  directly.
